Central Health Services, Ministry of Health and Family Welfare, Govt of India is the premier technical service of the Govt of India. The Central Health Services is manned entirely by Doctors having MBBS as the minimum qualification to enter the service. The entry to CHS is through a written examination and an interview conducted by the UPSC or union public service commission of India for Doctors with only MBBS degree. However, Doctors holding specialist degrees like MD/MS/DNB/MS/Mch which are doctoral and postdoctoral level degrees do not have to write an exam to get selected, they only need to sit for an interview conducted by the UPSC. The service is primarily concerned with patient care and management and hence these doctors are usually appointed as Assistant professors in the various hospitals under the central govt and they continue to serve as doctors till their retirement unless they get appointed to administrative posts like DGHS/Director general of health services, Additional DGHS or directors of various programmes like NVBDCP, Polio eradication programme or NLEP of the govt of India. The CHS doctors are the backbone of the ministry of health and they act as the highest technical advisor to the govt of India. The administrative side or the Secretariat of the ministry of health which is mainly concerned with looking after files and making necessary budgetary allotments can be manned by any central civil service officer, but primarily it is the IAS officers who look after the routine, non-technical side of the ministry. The CHS has 4 sub-cadres namely: 1) GDMO/General duty medical officer cadre 2) Specialists cadre- Non-teaching 3) Specialist cadre - teaching 4) Public health cadre. The promotions in CHS is time scale without linkage to vacancy via a scheme called DACP/dynamic assured career progression scheme where all doctors who join CHS right after their MBBS reaches the Joint secretary level at the govt of India( which is equivalent to the level of secretaries in the state govt) after completion of 20 years of service.

For Teaching cadre, the promotions are as follows:

Assistant professor(6600-grade pay) to associate professor(7600-grade pay) after 2 years of service Associate professor(7600-grade pay) to professor(8700-grade pay) after 4 years of service professor(8700-grade pay) to director-professor(10000-grade pay) after 7 years of service

Pay scales:

Entry Level: 5400-grade pay After 4 years: 6600 grade pay After 5 years: 7600 grade pay After 4 years: 8700 grade pay After 7 years: 10000-grade pay ( Joint secretary level at govt of India or Secretary at the state govt level)

The CHS is under the direct control of the officer in charge called as the DGHS/ Director general of health services who is a CHS doctor usually from the teaching cadre and is placed at the 80000 pay scale( secretary to the govt of India scale). The DGHS is assisted by Addl.DGHS and Special DGHS who are also CHS doctors placed at the apex scale of the govt of India at 80000 pay scale.

The secretariat is headed by the Health Secretary, Govt of India who is always an officer of the Indian Administrative Service(IAS) and is also placed at the apex scale of 80000 pay scale.

Doctors also get NPA(Non- practising allowance) @ 20% of the basic pay and NPA is considered as the part of the basic pay and not merely an allowance. However since cabinet secretary of India who is the head of all civil services draw a fixed pay scale of 90000, therefore doctors salary have been capped at the average of secretary and cabinet secretary pay at 85000 basic pay. Therefore, the highest officers draw the following pay scale:

DGHS/ADDL.DGHS/SPECIAL DGHS: 85000 basic pay Principal secretary ( Health): 80000 basic pay Cabinet secretary of India: 90000 basic pay High court judges: 80000 basic pay Supreme court judges and chief justices of high courts:90000 basic pay Chief justice of India: 100000 basic pay
