Trillium Woods is a Life Plan Community, or continuing care retirement community, in Plymouth, Minnesota that opened in 2015. Trillium Woods has 251 residents and is managed by Life Care Services, An LCS® Company. The community is located 15 miles west of downtown Minneapolis on 46 acres near Lakes Pomerleau and Camelot and adjacent to Three Rivers Park District and Northwest Greenway Trail systems.[1]

Services

Trillium Woods offers Independent Living with a range of living options both in apartments and patio homes..[1]

The Birches Health Center is located on the Trillium Woods property, has 44 private rooms and provides 24-hour emergency assistance by licensed health care professionals. Services include: Short-term rehabilitation care, long-term skilled nursing, memory care, and short-term respite care.[2]

Management Company

Life Care Services®, An LCS® Company, is the third-largest manager of rental senior living communities and Continuing Care Retirement Communities in the United States. Communities managed by Life Care Services may include Independent Living, Assisted Living, Memory Care for Alzheimer's disease and dementia diagnosed residents, rehabilitation, Skilled Nursing, or a combination of these living arrangements. Continuing Care Retirement Communities are communities where nursing care is administered on campus to prevent a resident from having to move to a new location to get care.[4]

Partnerships

Trillium Woods' management company, Life Care Services®, partnered with Lyft to bring on-demand transportation to a handful of their senior living communities. Currently, ten communities are taking part in the program and both companies hope to expand the service in the future. The ten communities utilizing Lyft are Sagewood in Phoenix, Arizona; The Heritage at Brentwood in Brentwood, Tennessee; Timber Ridge at Talus in Issaquah, Washington; North Oaks in Pikesville, Maryland; The Clare in Chicago, Illinois; Marquette in Indianapolis, Indiana; Blakehurst in Towson, Maryland;  Plantation Village in Wilmington, North Carolina;  Morningside of Fullerton in Fullerton, California; and Trillium Woods in Plymouth, Minnesota.[5]

Awards

Trillium Woods resident Rich Sachse received the 2018 Spirit of Aging Award from LeadingAge Minnesota. LeadingAge is a part of the Global Ageing Network (formerly IAHSA), which spans 30 countries across the globe. The Spirit of Aging Award honors an older adult who exemplifies the concept of positive and active aging. Sachse was selected from a pool of nominations submitted by a field of 750 senior care organizations throughout the state of Minnesota.[6]

Minnesota Star Tribune awarded Trillium Woods 2018 Top Workplace[7] and Plymouth Magazine awarded the community the Reader’s Choice Award in 2016[8], 2017[9], and 2018[10]
