The 1998 Capital Taxi Aereo Plane Crash was a scheduled cargo flight from Teresina-Senador Petrônio Portela Airport, Brazil, to Fortaleza-Pinto Martins Airport, Brazil. On October 21, 1998, an Embraer EMB 120 Brasilia, which took off from Teresina-Senador Petrônio Portela Airport, Brazil, crashed during final approach to Fortaleza-Pinto Martins Airport due to pilot error. All three occupants on the aircraft were killed. One person inside a brick house was also killed. Seven more people were injured. [1] It was Capital Taxi Aereo's first and only fatal accident.

Aircraft[edit]

The aircraft was delivered to DLT airlines in 1988. It made its first flight in 1988. The aircraft was equipped with 2 Pratt & Whitney Canada PW118 engines. The aircraft returned to Embraer in February 1990 and was briefly leased to Midway Commuter and Jetstream International. The aircraft was then moved to Selva in January 1997. [1]

Accident[edit]

On October 21, 1998, the Embraer EMB-120RT Brasilia took off from Teresina-Senador Petrônio Portela Airport, Brazil. At the time, it was carrying 2.5 tons (5,000 pounds) of medicine to Fortaleza-Pinto Martins Airport. During the descent for Fortaleza airport, the crew requested a visual approach with right base leg runway 13. The approach was flown at high speed. The flaps and land gear were selected at a very high speed. The unstabilized approach was not abandoned, and the thrust levers were positioned below the flight idle position. The airplane subsequently lost control and banked to the right, ripping through electrical cables and tearing across several rooftops before smashing into a brick house, killing a woman inside. Seven other people were injured in the resulting fires or flying debris. [1]

Investigation[edit]

The Centro de Investigação e Prevenção de Acidentes Aeronáuticos (CENIPA) immediately launched an investigation to the accident.

The investigation concluded that the following factors contributed to the accident: [1]

  1. There were signs of concern of the pilot with prolonged waiting caused by the local traffic in Fortaleza and by another traffic that was approaching the aerodrome. This accelerated the procedure of the final approach, with a descent at high speed and triggered other operational failures that led to the accident. In the same way, there were signs of excessive self-confidence by the pilot.
  2. The pilot misused the resources available in the operation of the aircraft, due to ineffective management of the tasks related to the pilot (pilot flying) and the co-pilot (pilot not flying) and the noncompliance with operational rules. The co-pilot did not advise the pilot during the approach and accepted the non-stabilized approach by the pilot. He also did not react to help the situation.
  3. The crew made the approach above the glide slope and at high speed. They thought inappropriately that it could proceed in those conditions and that there would be ways to reduce the speed and make the landing.
  4. The crew disobeyed the aircraft's operating rules by performing a destabilized approach outside the envelope recommended by the manufacturer and by placing the power levers below the flight idle position.
