A-10 surgery
A-10 surgery (Japanese: A点手術) is a minimally invasive facial reconstructive procedure developed by Japanese dentist, implant specialist, and orthodontic specialist Koji Yamamoto.
The technique is designed to address midface depression, alar base retrusion, and an acute nasolabial angle by fixing titanium plates at the anatomical Point A and the alar base, combined with soft-tissue augmentation using an artificial dermis.[1][2]
Development
The procedure originates from the Point A-Koji method, first reported by Yamamoto in 2022 as a technique that advances the subnasal soft-tissue point (Sn) through titanium fixation at Point A, the deepest part of the anterior maxilla.[1] In 2024, Yamamoto introduced a modified version of the procedure, called the modified A-10 surgery. This version incorporates a three-plate titanium system together with artificial dermis (Terudermis) to reinforce both midfacial projection and alar base thickness.[3] This modification allows three-dimensional correction of the midface and provides measurable improvements in facial convexity and nasolabial angle.
Technique
The operation is performed under local anesthesia using an intraoral approach. After dissecting the gingival mucosa along the maxillary buccal junction, pre-bent titanium plates are fixed between the lower edge of the piriform aperture and the maxillary alveolar bone. In the modified version, **three titanium plates** are employed — one central plate at Point A and two lateral plates near the alar base. An artificial dermis layer is inserted between the plate and bone to promote soft-tissue regeneration and maintain long-term volume.
Applications
A-10 surgery is used for aesthetic correction of midface flattening, alar base retrusion, and a narrow nasolabial angle, particularly in Asian patients whose facial structure tends to be less projecting.[2] It has also been discussed as a potential minimally invasive option for age-related changes such as piriform aperture enlargement and nasal base depression.[3]
Patent
The method was patented in the United States as a “surgical method for changing a nasolabial angle” based on titanium plate fixation at the maxilla.[4]
Academic recognition
The A-10 surgery and Point A-Koji method are cited in the reference text Maxillofacial Anatomy (Springer Nature, 2024) within the chapter "Ethnicity and Gender Differences." The chapter notes their relevance as minimally invasive approaches for altering midfacial skeletal profiles.[5]
