The Bank-A anomaly was a bank crisis.

An undisclosed bank automatically paid IRS $7.6 billion December 12 and 13, 2016. The bank borrowed $2.5 billion to avoid collapse. IRS returned the money to the bank immediately. The anomaly prompted a seven-month overhaul of the wire system. [1][2][3]
