Cosmetic Jaw Surgery

Genioplasty is a procedure that corrects the chin position. Frequently providing augmentation or greater projection (for people with weak chins). But it can correct asymmetry, and make a chin that is too prominent less so. Genioplasty can entail cutting and repositioning one’s own bone (osseous genioplasty), shaving down the bone, or utilizing an implant (silicone, polyethylene, PEEK, or titanium).[1] [2]

Jaw augmentation is considered for patients with an ill-defined jawline or lack of jaw angles. Typically this is achieved using a jawbone implant that can be off the shelf, or custom. These implants can be of various materials including silicone, polyethylene, PEEK, or titanium. Additionally bone graft materials can sometimes be used for jaw augmentation. Also less invasive “filler” like fat grafting, restylane, juvederm, and radiesse can be used for nonsurgical jaw augmentation.[3]

Malar or midface augmentation can also be performed to create more defined cheeks or upper jaw regions. The same materials or implants can be used to achieve this cosmetic balance. Fat grafting and non-invasive injectables can also enhance the upper jaw, cheeks, and midface.[4]

Jaw reduction in cases where areas of the jawbone are too large, prominent, or asymmetric. To soften or change contours of the jaw regions can be shaved down, contoured, or removed. This can be performed at the setting of orthognathic jaw surgery as well.[5]

V-Line surgery. This refers to a series of procedures, typically sought after in patients with wide zygomatic regions (cheek, malar), and wide and square mandibular angles, and a short, wide chin. The goal is to narrow the face progressively from the malar regions to the chin, creating the appearance of the V-line. These series of procedures are frequently sought after in Asian patients.[6]

Fat Grafting and Jaw Surgery. Fat injections from the patient at the time of jaw contouring, augmentation, or orthognathic surgery is an excellent cosmetic adjunct as well. Reports have shown that this improves the cosmetic result, and can reduce swelling, improve healing (due to growth factors, and adipose derived stem cells), and optimizes aesthetics.[7] [8][9][10][11][12]

Rhinoplasty and Jaw Surgery. The appearance of the nose can be altered due to jaw movements (principally the maxilla in a Le Fort-I osteotomy). In cases where the nasal function and appearance is enhanced, this is ideal. In other cases, either a nasal deformity exists to begin with, or a nasal deformity is created from the Le Fort-I osteotomy, a rhinoplasty may be required. This can be done either at the same time as cosmetic jaw surgery (simultaneous), or at some point later in time (staged).[13][14][15][16][17]
