Eliyahu Baal Shem of Worms

Rabbi Eliyahu Baal Shem of Worms was born in Krakow as the son of the scholar Yosef Jospa,[1] a Spanish Jew.[2] He arrived in Worms in 1590 and was one of the few rabbis to be given the Baal Shem title.[1][2]
