Jerry G. Blaivas

Jerry G. Blaivas, MD, FACS, is an American urologist.[citation needed] He is Senior Faculty at the Icahn School of Medicine at Mount Sinai[1] and Adjunct Professor at SUNY Downstate Medical Center.[2] [3] He previously served as Clinical Professor of Urology at Weill Cornell Medical College,[4] Attending Surgeon at New York Presbyterian Hospital[5] and Lenox Hill Hospital[6], and Professor of Urology and Vice Chairman of the Department of Urology at Columbia University College of Physicians and Surgeons.[7]

Dr. Blaivas is an acclaimed surgeon, academician, educator, author, and editor.[1] His clinical expertise ranges from the routine management of lower urinary tract conditions in men and women to the most complicated and difficult surgical problems.[1]

He is the founder and former Editor-in-Chief of the peer-reviewed journal Neurourology and Urodynamics[8] [1], and he is the Co-Founder of Symptelligence Medical Informatics, LLC[9] He has authored over 400 publications and has been listed by Castle Connolly as one of America’s Top Doctors in American and Top Doctors in New York Metro Area since its inception in 1992. [10] Recently, he was recognized by the Society for Urodynamics and Female Urology (SUFU) as one the founding fathers of female pelvic medicine and reconstructive surgery (FPMRS)[11][12] and received the Ferdinand. C. Valentine Medal from the New York Academy of Medicine for outstanding contributions to urology 2015.[13] [1]


Education

Dr. Blaivas is a graduate of Lawrence High School in New York (1960) and Tufts University from which he received a bachelor's degree in English Literature (1964) [1][7]. He received a medical degree from the Tufts University School of Medicine in 1968.[1] [4] After three years of General Surgery residency at Boston City Hospital (Tufts Division), he served in the United States Army as an orthopedic surgeon from 1971 to 1973.[1][7] He completed his urology residency at the Tufts New England Medical Center in 1976.[1][6]

Career

During his chief residency year he was awarded a grant from the National Multiple Sclerosis Foundation and upon graduation, he joined the Tufts New England Medical Center's faculty. He was appointed Director of the Urodynamic Laboratory which was funded by the multiple sclerosis grant.[citation needed] He was promoted to Associate Professor of Urology in 1979 and started one of the first post-graduate fellowship programs in what is now known as pelvic medicine and reconstructive surgery [1] [7] . In 1981, he joined the faculty of the College of Physicians and Surgeons of Columbia University, where he has held the positions of Professor of Clinical Urology, Director of Neurourology, Fellowship Director, and Vice Chairman of the department of urology.

In 1982 he founded Neurourology and Urodynamics, a peer review journal that subsequently became the official journal for the International Continence Society , The Society for Dynamics of the Upper Urinary Tract[14], the Society of Urodynamics and Female Urology and Urodynamic Society. [15] He was Editor in Chief from 1982 – 2006 and remains on the editorial board.[16]

In 1992, he left Columbia University and was appointed Clinical Professor of Urology at Weill Medical College of Cornell University.[4] He founded the UroCenter of New York in the same year.[5] [1] In 1998 he founded the Institute for Bladder and Prostate Research, a not-for-profit foundation dedicated to outcomes research in lower urinary tract and urogyncelogic disorders. [1] In 2010, Dr. Blaivas was appointed Adjunct Professor of Urology at SUNY Downstate Medical School and has been mentoring medical students, residents in urologic research and career path choices.[2]

In 2013, Dr. Blaivas and Stuart JKS Smyth co-founded Symptelligence Medical Informatics, LLC, a mHealth diagnostic, informatics and analytic company. [9]; which places him at the forefront of merging medicine and digital technology. In conjunction with Symptelligence Medical Informatics, he developed a software platform that enables patients to record their medical history on a smartphone app which he is using to conduct innovative research to develop clinical phenotypes that will enable precision medicine for patients with lower urinary tract[17] disorders.

Major Medical Contributions

The Institute for Bladder and Prostate Research

Blaivas founded The Institute for Bladder and Prostate Research (IBPR) in 1998 to address what he perceived as bias in medical research. The not-for-profit organization performs, sponsors and compiles scientific research about treatment options for lower urinary tract and urogynecological conditions that is not tainted by conflict of interest, research or publication bias.[18]

VideoUrodynamics

Blaivas was awarded a grant from the National Multiple Sclerosis Society to study the national history of voiding disturbances in multiple sclerosis. [19] This research allowed Blaivas to develop one of the first videourodynamic[8] units in the world, introducing a new method of simultaneously recording and displaying radiological images and urodynamic tracings on a single screen in order to obtain precise timing of events [20]. This technology was subsequently incorporated into commercially available Videourodynamics units. Prior to this, videourodynamics was commonly performed by recording physiological parameters on a strip chart recorder and X-rays on a fluorescent screen. It was therefore incumbent upon the physician to interpret the interrelationship of physiological and anatomical events by visualizing both images simultaneously on their respective video screens and noting the sequence of events.

When first introduced, urodynamics was mostly used as a method of classifying neurogenic bladder among patients with spinal cord injury. Blaivas' research pioneered use of urodynamics to diagnose subtle neurological disorders amongst patients without known neurological conditions.[12] He was later able to demonstrate the utility of urodynamics in treating patients with neurological diseases other than spinal cord injury (such as multiple sclerosis) [5], Parkinson’s disease [1], diabetes , and Shy-Drager Syndrome and for diagnosing the full range of lower urinary tract symptoms in men, women and children including urinary incontinence, urethral obstruction.

Classification of Lower Urinary Tract Disorders

Over the course of his career, Blaivas has been instrumental in devising classification systems for stress urinary incontinence[21] , detrusor-external sphincter dysynergia , overactive bladder[22] [23], urinary urgency[24], nocturia[25], bladder outlet obstruction in women. and urethral strictures.[26]

Blaivas documented a differential diagnosis of benign prostatic hypertrophy (BPH) that included prostatic obstruction, impaired detrusor contractility (weak bladder) detrusor over activity (bladder spasms) and sensory urgency (abnormal sensations). [27] Prior to the publication of his first paper on this issue in 1988, BPH and prostatic obstruction were thought to be synonymous. [26]

Autologous Rectus Fascial Pubovaginal Sling

Pubovaginal slings have been used for over a hundred years, but were only deemed appropriate for the most complicated cases. In the 1970’s Edward J. McGuire, MD, a Yale urologist, renewed interest in autologous slings, but at that time, one end of the sling was left attached to the rectus fascia. The net result was that there was a very high incidence of urethral obstruction because the unattached end was too short to reach the other side. Blaivas modified the technique by detaching both ends and creating a fascial graft, instead of a flap, that could be tensioned as loosely as necessary. Once the problem of causing obstruction was rectified, Blaivas began to promote the autologous sling as a primary procedure for what is now known as the “index patient”; which now has become the gold standard for sphincter incontinence treatment in women.[28]

Urethral Reconstruction in Women

Blaivas pioneered the techniques for urethral reconstruction in women. To date, he has published the largest series in the world and has performed over 140 urethral reconstructions.[29]

Overactive Bladder

Blaivas introduced the concept that overactive bladder should be considered a symptom complex with a differential diagnosis, rather than a syndrome[27] and investigated the characteristics of urinary urgency.[30] [24] The importance of this is to promote investigation into the remediable causes of OAB and to search for effective treatment, rather than the conventional knee jerk prescription of treatments with marginal success rates. He also developed the Urodynamic classification of Overactive Bladder. [22]

Symptelligence Medical Informatics

In 2013, Blaivas co-founded Symptelligence Medical Informatics that developed a disruptive software program designed to radically change the way health care is delivered and how research is conducted. The program, called weShare, is comprised of a mobile app for patient data entry, and doctor, patient and administrative portals.[31] [9]
