Prilosec OTC is an over-the-counter medication containing omeprazole, a proton pump inhibitor (PPI) used to reduce stomach acid. It is marketed for the treatment of frequent heartburn, defined as heartburn occurring two or more days per week. The product is distributed by Procter & Gamble in collaboration with AstraZeneca.[1]

History

Omeprazole was first approved by the U.S. Food and Drug Administration (FDA) in 1989 as a prescription medication for the treatment of gastroesophageal reflux disease (GERD) and other acid-related conditions.[2] In 2003, Prilosec OTC became the first PPI available without a prescription in the United States.[3]

Indications and Use

Prilosec OTC is indicated for individuals experiencing frequent heartburn. It works by inhibiting the gastric proton pump, thereby decreasing acid production in the stomach. The recommended regimen involves taking one delayed-release tablet daily for 14 consecutive days. Relief may begin within 24 hours, but full effectiveness typically requires one to four days. Consumers are advised not to exceed the recommended course without consulting a healthcare provider.[3]

The product is available in multiple formulations, including unflavored and wildberry-flavored varieties.[4]

Pharmacology

The active ingredient, omeprazole, is a selective and irreversible inhibitor of the H+/K+ ATPase enzyme system in gastric parietal cells. This inhibition suppresses the final step in gastric acid production. Other over-the-counter treatments for heartburn include antacids and histamine H2-receptor antagonists (H2RAs). Antacids neutralize existing stomach acid, while H2RAs reduce acid secretion through different mechanisms. Unlike PPIs, antacids and H2RAs typically provide shorter-duration relief.[5]

Marketing

Prilosec OTC has been promoted through various advertising campaigns, including a notable series featuring comedian Larry the Cable Guy during the 2010s. These campaigns emphasized the product’s ability to prevent heartburn when taken as directed.[6]
