SAGE-718 is a bioavailable oral derivative of 24S-Hydroxycholesterol, a main cholesterol metabolite within the brain that is also known as a neurosteroid. 24S-Hydroxycholesterol is a very potent positive allosteric modulator of the NMDA receptor. NMDA receptor hypofunction has been cited in a number of neurodegenerative diseases, Alzheimer’s disease, schizophrenia, Parkinson’s, and more. Overexpression of the NMDA receptor, specifically the NR2B subunit, has shown enhanced cognition in mammalian models, specifically rats. Clinical trials are testing its efficacy for cognitive enhancement in patients afflicted with Huntington’s disease, Alzheimer’s disease, and Parkinson’s disease; so far, results are promising.
