Main
- Wendi McLendon-Covey as Dr. Joyce[1], the Executive Director of St. Denis Medical and a former oncological surgeon who has big dreams for the future of the facility, dreams that are currently not realized.
- David Alan Grier as Dr. Ron[2], a defeated divorcee and an Emergency Department doctor who has accepted that the thrill is gone.
- Allison Tolman as Nurse Alex[3], the supervising nurse in the emergency department who is dedicated to providing excellent care to her patients, often sacrificing her personal life to maintain her high standards.
- Josh Lawson as Dr. Bruce[4], a trauma surgeon working primarily in the Emergency Department.
- Mekki Leeper as Nurse Matt[4], a well-meaning and newly hired Emergency Department nurse.
- Kahyun Kim as Nurse Serena[5], an Emergency Department nurse who leans toward homeopathy and is Alex's work wife.

Production
Development
The pilot was commissioned by NBC in August 2022, with production taking place shortly before the 2023 Writers Guild of America strike.[7] The series was given an official series order in June 2023.[6]
The series is produced by Universal Television, a division of Universal Studio Group, and is executive produced by series creators Eric Ledgin and Justin Spitzer, with Spitzer under his Universal TV-based production banner Spitzer Holding Company, and Simon Heuer, Head of TV Development at Spitzer Holding Company.[6]
St. Denis Medical has been moved to the 2024–25 season due to strike-related production delays.[8]
Casting
Upon the pilot commission, it was confirmed that Wendi McLendon-Covey would lead the series as Joyce.[1] In March 2023, Allison Tolman was cast in the pilot as Alex.[3] Later that month, David Alan Grier, Josh Lawson, Mekki Leeper and Kahyun Kim were confirmed as series regulars before production of the pilot commenced.[6][4]
