Stefano Cafarotti is an Italian thoracic surgeon known for developing innovative surgical treatments in oncology and congenital chest wall deformities. He coordinated and performed the world's first transplant of the main pulmonary artery to treat a thymic carcinoma..[1][2][3][4][5][6][7][8] He also developed a novel minimally invasive surgical technique for correcting pectus excavatum.[9]
Cafarotti is Chief of Thoracic surgery in Switzerland and serves as a professor at the University of Italian Switzerland (USI).[10] He is a faculty member in international training programs, including the Certificate of Advanced Studies in Lung Cancer, organized by the European School of Oncology and the University of Zurich, where he teaches advanced thoracic surgery for thymic and lung tumors.[11]
He is Clinical Director of the Lung Cancer Center of Southern Switzerland and Chair of the Board of the Multidisciplinary Oncology Centers in the Canton of Ticino (prostate, breast, neuro, colorectal, lung), serving as the medical lead for the General Directorate of the Ente Ospedaliero Cantonale (EOC).[12]
According to Cancer World, Cafarotti was among the first European experts to implement and propose cancer patient management protocols during the COVID‑19 pandemic.[13][14] The Italian-speaking regions, including Ticino, were among the first in Europe to face the pandemic.[15]
Education and career
Cafarotti graduated cum laude in Medicine and Surgery in 2005 from the Università Cattolica del Sacro Cuore and completed his residency in thoracic surgery at Policlinico Agostino Gemelli in 2012 with top marks.
He later practiced in Switzerland, where he obtained his license in heart and thoracic vessel surgery and the FMH title in thoracic surgery, as well as the title of university professor.
In Belgium, he earned the European Fellow of the European Board of Thoracic Surgery (FEBTS) awarded by the European Union of Medical Specialists.[16]
In 2019, he became Head and later Chief of the Thoracic Surgery Unit at the Ente Ospedaliero Cantonale in Ticino. He also serves as Director of the Lung Cancer Center of Southern Switzerland and Chair of the regional oncology board.[17]
In 2024, he joined the Scientific Committee of the Brain Disease Foundation, which funds research projects on brain tumors.[18]
He is the founder of the non-profit organization Ticino Alliance for Lung Cancer Advocacy, Support and Education (ALCASE Ticino).[19]
Scientific and clinical activity
Cafarotti is the author of numerous scientific publications and has over 1,200 citations according to Google Scholar (as of April 2025).[20] He is the editor of a manual on therapeutic strategies in lung cancer surgery,[21] and a review editor for international journals.[22]
Several of his studies have been cited in international medical guidelines.[23]
Between 2024 and 2025, he serves as a member of the publication selection committee for the World Conference of the International Association for the Study of Lung Cancer (IASLC)[24] and is also part of the IASLC Patient Advocates Committee[25].
From a clinical standpoint, in 2019 he introduced a new minimally invasive technique (via a single 2 cm thoracic incision) for the correction of pectus excavatum[9]. In 2022, together with T. Torre, M. Patella, and S. Demertzis, he coordinated and performed the world’s first radical thymectomy with pulmonary artery transplantation from a cadaveric donor in a patient affected by previously deemed inoperable thymic carcinoma[26] [27]. The procedure was considered by the *Journal of Cardiothoracic Surgery* as a major advancement in the treatment of thymic tumors[1].
He also led the first pilot study in Italian-speaking Switzerland (second in the country) on low-dose CT lung cancer screening, detecting 44% of asymptomatic thoracic pathologies[28][29].
Awards and recognition
As president of the board of directors of the oncological centers network, he led the Canton of Ticino to achieve European Cancer Center certification for colorectal cancer[30], prostate cancer[31] and lung cancer[32], while also confirming the EUSOMA certification for breast cancer care[33]
