Mileska's advocacy focuses on gender equality, sexual and reproductive health and rights, menstrual justice, youth participation and the prevention of violence against women and girls.[2]

Advocacy

Mileska began her activism in the areas of sexual and reproductive health and rights and gender equality. In 2019, UN Women profiled her as part of its Generation Equality campaign, highlighting her work advocating for sexual and reproductive health and rights.[3]

She has subsequently focused on gender equality, menstrual health and youth participation in decision-making.[1]

PERIOD Skopje

Mileska is a co-founder of PERIOD Skopje, a youth-led initiative focused on menstrual health and menstrual justice in North Macedonia.[1]

The initiative works to challenge stigma surrounding menstruation and promote access to menstrual-health information and products. Its work connects menstrual justice with broader questions of gender equality and social inclusion.[2]

Youth leadership

Mileska has participated in European and international youth initiatives addressing gender equality, violence against women and girls and youth participation.

She served as a youth ambassador for Women Against Violence Europe (WAVE), contributing to youth advocacy against violence against women and girls. Her two-year mandate concluded in 2024.[4]

She has also worked as an educator, using workshops and youth-led educational activities to promote gender equality and awareness of sexual and reproductive health and rights.[3]

United Nations Young Leader for the SDGs

In 2025, Mileska was selected as one of the 17 Young Leaders for the Sustainable Development Goals for the 2025–2027 cohort of the United Nations Youth Office.[5]

The programme recognizes young people whose work contributes to the Sustainable Development Goals. Mileska was selected for her work promoting gender equality, menstrual justice and youth participation.[1]

As a Young Leader for the SDGs, she participates in international advocacy and youth-engagement activities concerning equality, peace and sustainable development.[2]

Peace and justice advocacy

Mileska has connected gender equality with broader questions of peace and justice. In a 2026 profile, UN Women described her work in terms of challenging the idea that peace is simply the absence of conflict, emphasizing instead the importance of justice, equality and participation.[2]
