Founded by Jamie Thomas, Cognito Forms was launched in 2013 after 3.5 years of planning and development.[1] The company is privately held and based in Columbia, South Carolina.[2]

Software and Services[edit]

Cognito Forms has a drag and drop interface that includes calculated fields, file uploads, conditional logic, repeating sections, payment fields, and the ability to generate PDF and Word documents from form entries.[3] Once form data is collected, Cognito Forms users can view each form entry in a spreadsheet-style list, and filter or search for entries based on specified criteria.[4] The platform also integrates with Microsoft Flow and Zapier to enable integration with several hundred other web applications and services.[5][6]

Cognito Forms offers the option to encrypt entry data at rest, as well as HIPAA-compliance for users in the healthcare industry.[7]
