Greater Family Health is a non-profit Federally Qualified Health Center founded in 1998 in Elgin, Illinois[1]. Prior to May 24, 2021, the organization was known as Greater Elgin Family Care Center[2].
Mission
The mission of Greater Family Health is to provide quality, affordable health care for all, including those without the ability to pay[1]. The organization provides medical, dental, behavioral, and LGBTQ health care services to low-income individuals and families[3]. In 2021, of the 58,808 patients served by the organization, 81.46% identified as racial and/or ethnic minorities, 36.77% were best served in a language other than English, and 90.08% had income at or below 100% of Federal Poverty Guidelines[4].
History
In 1998, a coalition of local stakeholders and organizations was formed to address the health care needs of Elgin’s low-income community[1]. Known as the Community Task Force 2000, their objective was to provide needed primary care services to Elgin residents and minimize the use of local hospitals' emergency rooms for non-emergency needs[5]. As a result of the Task Force's work, Greater Family Health was incorporated on August 27, 1998[6]. In 2002, the organization successfully applied to become a Federally Qualified Health Center and thus began receiving support from the Bureau of Primary Health Care[1].

Outcomes
In 2022, Greater Family Health was recognized as a Health Center Quality Leader - Gold for its clinical quality measure performance among all Federally Qualified Health Centers[4]. The "Gold" designation indicates that the organization was ranked in the top 10% of all reporting Health Centers[8].
