Included Health is a U.S.-based healthcare company that provides a combination of virtual care, navigation, and communities-based healthcare services for individuals across the country. As of late 2023, it served tens of millions of individual members across America, counting as clients 32 of the Fortune 100 companies.[1]

The company has evolved through a series of innovations since its founding as Grand Rounds Health in 2011, which focused on expert medical opinions. Following the merger of Grand Rounds Health and Doctor on Demand in 2021, the company has expanded to offer virtual care delivery, healthcare advocacy and navigation services, and virtual-first specialty care clinics.[2][1] The merger allowed the company to provide personalized, integrated virtual care services for more people, specifically underserved or marginalized communities.[1]

The merged company acquired legacy company Included Health in 2021 and rebranded under the Included Health name.[3]

Their platform, All-Included Care™™, was brought to market in 2022, and combines virtual and in-person care with expert care guidance and advocacy, addressing physical health, mental health, and financial concerns associated with healthcare.[4][5][6] Included Health seeks to leverage virtual-first care delivery, AI-powered technology, data science, and clinical expertise to bridge gaps between patients and healthcare providers.[7]

The platform is used by a range of organizations, including major employers like Walmart and Salesforce, health plans, and the public sector, such as the California Public Employees' Retirement System (CalPERS). They are located in San Francisco, California, and led by CEO and co-founder, Owen Tripp.[citation needed] To date, they have raised $508 million.[8]

History

Pre-Merger (2011 – 2021)

Included Health was founded under the name Grand Rounds Health (“Grand Rounds”) in 2011 by Owen Tripp and Dr. Lawrence “Rusty” Hofmann with the mission to raise the standard of health care by becoming a single source where all of a patient’s healthcare services are provided and patients are supported physically and financially.[9][10]

Tripp and Hoffman’s founding of Grand Rounds was driven by their own personal health experiences. Dr. Hofmann’s son was diagnosed with a life-threatening condition, for which he was able to coordinate expert care that many others would not have access to. Tripp experienced inefficiency and lack of access to experts in the healthcare industry when he lost a relative to a rare cancer that likely could have been effectively treated elsewhere. These experiences highlighted a gap in healthcare access that inspired Tripp and Hofmann to found the company and aim to democratize access to medical experts.[11][12][2]

As Grand Rounds, the company, headquartered in San Francisco, raised $272 million, highlighted by a Series E round led by The Carlyle Group.[13]

In March 2021, it was announced that Grand Rounds and Doctor On Demand were merging in a deal that would form a multibillion-dollar entity. Doctor On Demand at that point had raised more than $235 million. The combination of the two had raised funds over $500 million.[14] As part of the deal, Owen Tripp was named to run the combined company.[14] The newly-formed entity was positioned as the first integrated company of its kind to provide end-to-end primary care, specialized care, chronic condition management, and behavioral health under a single platform.[15][16][17]

Shortly after this in May 2021, the two entities purchased Included Health, a care platform focused on LGBTQ+ communities, signaling its commitment to inclusive and integrated care.[18] Later that year, the two entities rebranded, adopting the legacy Included Health name to demonstrate a diverse array of services.[3][5]

Post-Merger (2021 – present)

In December 2021, Included Health announced that it was targeting an IPO for the first half of 2022.[8] Given worsening market conditions, it remained private.[19] Following this decision, Included Health has seen double-digit revenue growth.[19]

Included Health launched All-Included Care in November 2022, a new product combining virtual care, access to in-person care, and patient navigation.[6]

In early 2024, Included Health announced it had embedded the CDC’s “Healthy Days” patient-reported measure into its care navigation platform following a 2022 pilot involving over 23,000 individuals. The pilot reportedly led to an average increase of two healthy days per month among participants and resulted in strong engagement with follow-up care services.[20]

Client examples

Included Health has served as the primary virtual care partner for Walmart’s employee health plans since 2016, supporting associates and their families with comprehensive virtual primary care coverage across 49 states. The partnership expanded from initial pilots in three states to nationwide coverage by 2023, offering services such as mental health care, physical therapy, digestive health, and at-home lab testing without employee co-pays.[21][22]

Outcomes from Walmart’s multi-year pilot included a 24% reduction in average HbA1c among diabetes patients, a 14% reduction in blood pressure for hypertension patients, and an overall 11% decrease in total cost of care—underscoring the model’s value in improving chronic disease outcomes and access for employees, especially those in primary care shortage regions.[21][23]

Salesforce is partnered with Included Health to centralize benefits for over 83,000 employees and provide access to top clinicians, billing support, and an accessible member experience. The company also rolled out gender inclusive benefits, and counseling services through Included Health.[24]

AT&T partners with Included Health to offer employees a virtual healthcare platform that provides whole-person care through integrated primary, mental health, and pharmacy services.[25]

One of the companies Included Health serves in the public sector is the California Public Employees' Retirement System (CalPERS) by offering healthcare and navigation services for public employees, retirees, and their families.[26][27] In 2024, Included Health partnered with Blue Shield of California to provide integrated health care and navigation support for approximately 400,000 CalPERS PPO members under new contracts starting in the 2025 coverage year, with financial incentives tied to cost control and quality improvement targets.[28]

Products

Included Health’s All-Included Care delivers personalized, all-in-one support for mind, body, and wallet—combining virtual primary, urgent, and mental health care with 24/7 guidance on insurance, benefits, and billing. [29][6]

Doctor On Demand by Included Health** delivers integrated virtual healthcare services and provides real-time and scheduled access to U.S.-licensed medical and behavioral health providers.[30] Doctor On Demand’s virtual care services expanded during the COVID-19 pandemic, more than doubling its covered lives to 98 million and delivering over 3 million virtual visits in the first half of 2020.[31]

Grand Rounds’ merger with Doctor On Demand in 2021 combined data-driven clinical navigation and patient advocacy with integrated virtual healthcare to create a multidisciplinary care platform.[1]

Research and insights

In 2021, Grand Rounds Health collaborated with several major employers—including Walmart, Target, Best Buy, Medtronic, and State Farm—to launch the **Black Community Innovation Coalition**, a culturally tailored telemedicine initiative aimed at serving Black American employees through concierge-style care navigation and advocacy.[32]

The program was designed to improve trust and equity in healthcare access by addressing disparities through targeted outreach and provider matching informed by market research and employee focus groups.

Included Health's 2023 Workforce Vitals Report, based on a survey of 2,500 employees, identified concerns related to physical and mental health, as well as economic stressors affecting workplace well-being.[33]

In January 2025, Included Health participated in a study around the impacts of a virtual care navigation service on member-reported outcomes among gay, lesbian, bisexual, transgender, and queer populations.[34][34]

In May 2025, the company published a study revealing that financial anxiety related to healthcare costs is at an all-time high and that most Americans are proactively addressing mental and physical health holistically by integrating habits like exercise, sleep, and social connections into their routines.[35] The survey of 2,000 people found that 70% think that their mental health has a direct impact on their physical health.[36]

The company also conducted a study analyzing changes in depression symptom severity among 2,320 patients receiving virtual behavioral health care between 2019 and 2020. The study found that over 40% of patients in psychiatric care achieved full remission after four visits, while 42% of those undergoing behavioral therapy reached remission after 8 visits. The results indicated significant improvements in depression symptoms, suggesting that virtual care can be an effective option for managing depression.[37]
