Raziya Foundation Eye Hospital (Urdu: رضیہ فاؤنڈیشن آئی ہسپتال) is a secondary care hospital located in Rawalpindi, Punjab, Pakistan. Which is made with the aim to prevent and control blindness by providing best eye care services at very affordable prices. Their main goals are awareness, education and prevention.

Dr. Hammad Tariq (Urdu: ڈاکٹر حماد طارق)[2] is currently serving as Administrator of Raziya Foundation Eye Hospital. He is running the Primary Eye Department of the hospital. He is qualified doctor of optometry.
