The medal pendant is of bronze, octagonal in shape, 1-1/4 inches long and 1-1/4 inches wide. On the obverse side of the pendant is an outline of the borders of the State of Texas, with the words, "TEXAS STATE GUARD" on the upper three sides of the octagon and "ACHIEVEMENT" along the lower three sides of the octagon. The word “ACHIEVEMENT” is bookended by a waterdrop with point facing upwards, atop a circle, atop a waterdrop with points towards the word “ACHIEVEMENT”. On the reverse side of the pendant is a five-pointed raised star, one point up, 3/8 of an inch in diameter surrounded by a wreath formed by an olive branch on the right and a live oak branch on the left, surrounded by the words "TEXAS STATE GUARD" along the upper three sides of the octagon and "TEXANS SERVING TEXANS" along the lower three sides of the octagon, in raised letters. [3]

The pendant is suspended by a ring from a rayon moiré ribbon 1-3/8 inches long 1-3/8 inches and wide, composed of three vertical stripes of Bluebird #67117 (9/16 inch), Ultramarine #67118 (1/4 inch), and Bluebird #67117 (9/16 inch).[3]
